St. Therese partners with North Memorial for transitional care center

St. Therese has opened Transitional Care by Saint Therese to provide specialized care to North Memorial Medical Center patients preparing to return home.

Located on the fourth floor of North Memorial Medical Center in Robbinsdale, 32 private rooms are designated for patients transitioning from hospital to home due to illness, injury or surgery.

According to St. Therese, the partnership with North Memorial aims to improve patient satisfaction and decrease the length of their hospital stay for a variety of conditions.

“Patients benefit from a continuity of care by being connected to North Memorial Medical Center and may receive regular visits from specialized physicians and nurse practitioners, on-unit physical, occupational and speech therapists, registered dietitians and pastoral care staff,” a St. Therese press release stated. “The latest technologies are being utilized to provide the very best in patient care including: a two-way nurse call system, an automated medication dispensing system and on-demand, in-room education.”

The Catholic nonprofit also provides senior housing and services in New Hope, Brooklyn Park, Shoreview and Woodbury.
